When ketosis actually starts in a fast

Ketosis is not a cleanse. It is a fuel shift: the brain and heart running mostly on ketones after glycogen is gone. For most people that is closer to 24 hours, not hour 12 and not a pill.
The sequence
- 0–4h — Anabolic. Digesting. Insulin high.
- 4–16h — Catabolic. Liver glycogen.
- 16–24h — Fat burning. Fat becomes the main fuel. Ketones begin to rise. This is the 16:8 payoff, still short of full ketosis.
- 24–48h — Ketosis. Ketones are the main fuel. Hunger often drops. Cellular cleanup (autophagy) speeds up.
- 48–72h — Deep ketosis. Only if you already know what you are doing, ideally with a clinician in the loop.
- 72h+ — Extended. Same warning, louder.
If an app or a video says you hit ketosis at hour 8, they are flattening a range into a sales beat.
How this shows up
Some people notice a clearer head or quieter hunger. Some notice nothing. Breath can change. Blood BHB is the honest measurement if you care. Urine strips lag.
